Reimbursing Hospital Bills from Philhealth – Requirements
I wasn’t able to use my Philhealth when we settled my hospital bills last week. No worries from me since I can still reimburse from them itself by submitting the requirements within 60 days of hospitalization. What are the requirements?
- Philhealth Form 1 – for employed individual which is to be signed by the company
- Philhealth Form 2 – which is to be signed by the Philhealth office at the hospital and by the doctor
- Official Receipt of the bills you have paid issued by the hospital
- Official Receipt from you doctor for the fee if it isn’t included in the hospital bills
- Medical Records if operation or other medical procedures were performed
- Philhealth ID or Member Data Record which could be obtained from the Philhealth itself.
I do hope I can reimburse again, just like last time when I gave birth. It will take months but a least, I received it before. Hope it will be like that again.
